No problem setting up the LLC for an Italian and there is no information exchange with Italy either. Even better as long as you don't sell or operate in the US then US LLC is 100% tax free.
As far as Malta you would open a bank account and wire or PayPal money to the account or us a bank debit card from elsewhere in the Eurozone to withdraw money or transfer to the Maltese account. Keep in mind with residency in Malta you won't have to worry about other EU countries unless you work there or spend over 183 days a year.
A lot of Italians opt for virtual residency in the UK quite what they do and how they structure things I am not sure but we have 72 Italian clients with a UK address, UK cellphone and UK landline number. We can even arrange for UK landlines to ring anywhere worldwide with a UK ringing tone.
